The Arizona Center for Fertility Studies (ACFS), one of Arizona's first IVF programs when it was established in 1981, formally merged with Arizona Associates for Reproductive Health in 2021 and the combined entity joined the CCRM Fertility Network in 2024, uniting a four-decade local legacy with a North American research network. This record represents the legacy ACFS entity; active clinical services are now provided through the merged CCRM-network practice in Scottsdale.

Accepted insurer candidates
ACFS merged into AZARH (Arizona Associates for Reproductive Health). AZARH has a dedicated insurance participation page listing all accepted carriers. Their lab is contracted with all major insurance companies. Insurance acceptance is self-reported by the clinic and changes frequently. Fertility benefits also vary by individual employer plan, and self-funded plans may opt out of state mandates.
