About
ACFS Fertility, Arizona Associates for Reproductive Health is the Scottsdale practice now operating as CCRM Fertility of Arizona, part of the CCRM Fertility Network. Dr. Ketan Patel, who founded Arizona Associates for Reproductive Health in 2005, serves as Co-Medical Director and practices at both the Scottsdale Main Center and the Gilbert location. The practice was formed when AZARH merged with the Arizona Center for Fertility Studies in 2021, and the combined ACFS joined CCRM in 2024. Clinical services include IVF, IUI, egg freezing, donor egg, gestational carrier coordination, PGT, and reproductive surgery, with documented focus areas in PCOS, endometriosis, and LGBTQIA+ family building. Dr. Patel completed his REI fellowship at UC San Diego and previously served as Medical Director for Reproductive Endocrinology and Infertility at Mayo Clinic. CCRM lists him as a Phoenix Magazine Top Doctor for 20 consecutive years.

Cost factors to verify
A quoted price rarely tells the whole story. Ask the clinic how each of these is handled before comparing prices:
- Diagnosis and testing — workup before treatment starts
- Medications — injectables and pharmacy costs
- Monitoring — cycle bloodwork, ultrasounds, and visits
- Lab and anesthesia — embryology lab work, retrieval, and outside lab fees
- Embryo testing — genetic testing (PGT) when used
- Donor services — donor eggs, sperm, or gestational carrier coordination
- Insurance plan design — coverage, network rules, and benefit limits
- Financing — payment plans, multi-cycle packages, and refund programs
Questions to ask this clinic
- Which services happen at this location, and which require travel to another lab or partner site?
- Which fees are included in the quoted cycle price, and which are billed separately?
- How should patients verify insurance benefits before starting treatment?

Insurance
Accepted insurer candidates
AZARH has a dedicated insurance participation page listing contracted carriers. Insurance acceptance is self-reported by the clinic and changes frequently. Fertility benefits also vary by individual employer plan, and self-funded plans may opt out of state mandates. Always verify coverage directly with the clinic and your insurer before starting treatment.
