Coverage Details

Pennsylvania has no broad state mandate requiring private plans to cover infertility treatment, IUI, or IVF, and KFF lists both private infertility treatment and Medicaid IVF as not covered by a Pennsylvania fertility mandate. Patients may still have voluntary employer fertility benefits, a negotiated rider, or coverage for evaluation of an underlying medical condition, but that is plan design rather than a statewide mandate-to-cover or mandate-to-offer rule. Proposed legislation has been discussed, but current launch copy should treat IVF coverage as plan-specific and subject to ERISA self-funded-plan limits.

Self-Funded Employer Plans (ERISA)

Self-funded employer health plans are generally governed by ERISA and may be exempt from state insurance mandates; verify benefits with the clinic and insurer.
