Minnesota has no private-market infertility mandate; Chapter 62A contains no infertility-coverage requirement, and the diagnosis flag previously shown traced to the Medicaid column rather than a private-insurance law. Medicaid context covers counseling for and diagnosis of infertility and related services, but not covered: IUI, IVF, fertility drugs and related services, and sterilization reversal. State mandates vary by market and plan type and generally do not control self-funded employer plans.

Self-Funded Employer Plans (ERISA)

Self-funded employer health plans are generally governed by ERISA and may be exempt from state insurance mandates; verify benefits with the clinic and insurer.
