Louisiana has no general infertility diagnosis or treatment mandate. RS 22:1036 is an anti-exclusion rule, not a diagnosis mandate: it prevents insurers from excluding coverage for a correctable medical condition merely because that condition results in infertility, where the condition would otherwise be covered. The state's affirmative mandate is preservation only and cancer-only: RS 22:1036.1 requires coverage of standard fertility preservation services for cancer-related iatrogenic infertility, excluding self-insured and religious employers.
