Employer benefit program candidates
FCLV is contracted with some insurances but specific carriers not listed publicly. Granite Sahara Surgical Center and Ovation Fertility labs not contracted with any insurance. Nevada has no IVF mandate. Insurance acceptance is self-reported by the clinic and changes frequently. Fertility benefits also vary by individual employer plan, and self-funded plans may opt out of state mandates.
